Here at Croydon SocialP, we see the value in everyone. We want to be a catalyst for positive change in people's health and wellbeing, and since April 2017, we’ve been driven by the same ideas our organisation was initially founded. Building upon 3 basic principles; engagement, opportunities and support.
The Croydon SocialP team is around to help progress, empower and coordinate opportunities. Finding the best ways of engaging with patients and the wider Croydon community, developing an array of locally based programmes proven to assist in their health self-management and supporting them through the transition of long-term behavioral change. These can be direct referrals to the programmes from GP’s or self-referrals from any organization or individual.
